Assessment of the member's medical, social, educational, and other needs. This intake process will include the results of the psychological evaluation; developmental and biographical history of the member's behaviors, traits, and qualities; the member's current circumstances; the resources, services and accommodations provided the member; the member's significant relationships, problems, requirements, and needs.

The assessment will be coordinated by the individual support coordinator in consultation with other professionals, providers, family or guardians, and the member, as necessary.
